1826 is 5 x 365 + 1. That is the number of days in the 5 years (assuming only one leap year in that time) that is used to identify cancer survivors.

My take is that the lyrics are not about the cancer survivor but the cancer care giver. After 5 years the care giver is exhausted with the constant demands and the necessity of always being pleasant. This is especially trying when the cancer victim is particularly demanding and unpleasant.

After 5 years it is socially acceptable for the care giver to chuck it all and run away. The cancer victim has matriculated to cancer survivor.
