My opinion is that the singer or the character she is singing about, lost her loved one because of something they did and couldn't forgive themselves for. She talks about innocence and faltering, and faltering is starting to lose strength or momentum. She's trying to explain tot he person that they should forgive themselves. In the beginning of the song, she says that she couldn't love him or this person in the "right" way, and that she had been loving him in her own way which could have been good or bad. She may not be able to forgive them either, but she believes that they should forgive themselves because it wasn't entirely their fault.

All we are is broken glass
Thrown to the floor, we were never meant to last
And all we are, are empty shells
Try to pick us up, you're gonna cut yourself
He, or they, believe that they are broken glass, thrown to the floor and rejected. They are empty like shells, but, if you mess with them you will get hurt.

Oh, the prettiest of lights that hang the hallways of the home
I believe this line is talking about something similar to the first verse where it said, "It's not what you painted in my head, there's so much there instead of all the colours that I saw." This line possibly is referring to something like the colours, or, something about the sorrow and he wants to just light his way home somehow.
