Personally I think this song is about jon going through an internal/mental battle with himself, probably because all the drugs messed up is mind. I feel like I picked up on the meaning instantly because this is something I'm personally going through, and can relate to. Maybe I'm wrong but you can take the lyrics however you like. "Who drains the blood from my head? I do! I do! Who comes through with the meds? I do! I do" Draining the blood from his head meaning the meds drain out all the negative thoughts he has in his head. "Who accepts me as I am? I do! I do!" I think it means he loves himself only when he's high on meds "Got a good education on disinformation, Adept at constructing lies" This is my favorite DGD lyric ever. I think it means his mind is so messed up from the drugs, he only has that negative voice in his head. By saying "education" I believe its something he's been dealing with for 10+ years. "the crack in the staircase i smoked out of boredom" "I'm the one, I'm the one With my hands around the gun! I am not afraid, I am not afraid." It's a metaphor, his hand around the gun meaning He's the one killing himself with the drugs. So what, I'll die alone, I am not afraid. I think the outro is just some nonsense he came up with while tripping lol.
