It starts off with him getting lucky in the park with a call girl, as he pumps his way into his hat (condom) or so he thought. Until it hit the ground, then it became apparent that it had come off. The boulder on the shoulder is a reference to him coping a feel, "over the shoulder boulder holder" is another name for a bra. He saw the light when a cop car pulled up (spot light), tripped over the merry-go-round as he was making a break for it, (that's where the running like an Indian comes into play) cuz he was wearing his native suit. Him getting the sneezing and wheezing was cuz he caught the gonna-a-syphilis from the hooker he was getting freaky with. The silicone sister and her manager mister (pimp) were now also looking for him, so he's running again. The curly wurly is his mustache as he got a jolt from the early perly (aka bugger sugar) and with that he was ready to ride the lightning. The following stanza is where he's in the doctors room getting his check up, blinded by the doctors light, hit in the funny bone, his escapades have landed him here, where the fun's at. And him catching an STD, is made apparent when he sees the girls dancing on the corner he gets a quick reminder of the cold moonstone (cuz he had just got a penicillin shot to the butt) which, if you were in the military you got in boot camp, or in his case from the gonna-a-syphilis, it definitely feels like a cold stone on your moon. In the service, we called it a peanut butter shot, cuz it feels like a big cold blob of pb in your butt check, which brings us back to the beginning. The madmen being a foreshadowing for the syphilis, cuz if left untreated will make you go mad, like Howard Hughes, and the drummers bummers (or the drips mips form the gonna-a-syphilis), well you get it.
