The song is performed in the first person as the patient as he tries to convince his doctors that he knows more about what he has than the doctors are telling him. Although not stated in the song, it is told through a series of audio tapes and video recordings that the patient was given access to.

As he sleeps, that's when he feels the safest as his mind doesn't allow the other him come out during that time. He hides under his blanket as a safety barrier as he fees every little bit will help him. This keeps him away from lights and bright objects from getting in his eyes which would cause him to wake up which could cause his demon side to take over.
