I believe this song tells the story of how David was trapped in his own mind when trying to get over the death of his deceased girlfriend. References to the fear of dreaming comes from his own experiance of terrifying dreams about her. Unlike inside the fire, which deals with him trying to accept that she is dead, asylum is about him trying to move past his own psychological "asylum". Very intrecate song, truly a masterwork.
