All I can think of when hearing this is a girl is driving on the highway, speaking to her best friend and boyfriend on the phone. Her boyfriend had told her to meet him somewhere and was planning to propose, and the best friend was in on it. The thing was, the girl thought the boyfriend was going to break up with her because he'd been so distant planning the proposal, and the girl had turned to alcohol. Not enough to be visibly and audibly drunk, but enough to be slightly impaired, and talking on the phone wasn't helping. Of course, neither the best friend nor the boyfriend know this. When the girl crashes her car, the best friend and boyfriend don't know what's going on, and keep asking her to say something so they know she's okay. I imagine this as a duet between the best friend an boyfriend at the girl's funeral, going over the events and the fear. Essentially apologizing, because the boyfriend thinks it's his fault for being distant, and the best friend thinks she should've noticed the slight slur in the girl's words, the rumble of the car, and prevented anything from happening. I doubt this happened to the writer, but it makes me think of this.
