If his heart was still alive it would surely break means that he no longer can feel anything he's numb and he is simply allowing his body to join his heart in death. But he knows by ending his pain will cause pain to the ones he loves. So he wants to tell them that only his body is gone he is still alive within them. Smile when you think of me my body's gone that's all. Remember the good times, keep me alive.
