I think it is about losing his love to someone else and the anger that consumes him.
'tell how that you can swim when ropes are wrapped around you limbs' makes me think the girl he lost angered him.
'i cut the throat of betrayal to watch it bleed' makes me think he knew who ever the girl he lost was with.
'a poetic sonnet passion that I scream to the sky' makes me think he still loves the girl but is angry she has left him.
The rest I think is not with the girl, but with another person, trying to get over her.
And then the last part is I think his words to the girl he lost.
